Using a metered-dose inhaler (pump inhaler) during the fast is a matter of genuine scholarly disagreement:
- Majority position: It breaks the fast, because the medication enters the airways and is considered a substance entering the body through an open passage.
- Minority position (also held by respected contemporary scholars): It does not break the fast, because the amount reaching the stomach is negligible and the medication is not nutritive.
The practical conclusion for someone with genuine medical need: If you must use your inhaler to breathe safely, use it. Your health comes before the fast. You can make up the day (qada) after Ramadan. Your life is protected by Islamic law at a higher priority than any act of worship.

Standard relief inhaler (salbutamol/albuterol MDI) This is the most common inhaler type. Majority: breaks the fast. Minority: does not. For someone who only needs it occasionally (once or twice during the fast day), the minority position is the most liveable and is held by qualified scholars.

Nebulizer Scholars who analyzed nebulizer use specifically โ including the Islamic Fiqh Academy โ generally concluded it breaks the fast due to the larger amount of substance reaching the airways. The medical necessity exception applies.
Dry powder inhaler (DPI) Same analysis as MDI. The delivery mechanism differs but the substance entering the body is comparable.
Oxygen through a mask Pure oxygen is not a substance that breaks the fast. Using oxygen while fasting does not affect the fast's validity.
